The Kids Movie Day

The Daily Cares Presents "The Kids Movie Day," where our team sponsors kids and teens (and sometimes one parent/guardian) to a complete moviegoers experience to one of their favorite new movies. We understand that a lot of us have fallen on hard times and can't do all the things you may want to do with your child. Our team would like to be there for you and offer these exciting things to you. Filling out this form is a request for a spot. This is not guaranteed. We will do our best to try and accommodate all requests.
All children/teens must be 17 years of age or under. Kids under 14 years old must accompanied by one parent. Your information will be protected and secured, before and after you've filed this form.
We will only use this in order to text you the time and location of your movie, as well as to confirm your snack options.
If you are apart of a group or team invited to attend movie day, please write N/A. If you are over 14 years of age and will not attend with a parent, please still put in their information, as they will be emailed a consent form.
